Notice of Annual Termite Inspection
We are sending you this letter to inform you that your annual
termite inspection is due on your home. In order to renew your contract the inspection must be completed on or before _______.
If you would, upon receiving this letter call our office to schedule your appointment. We will call once in the morning, once
in the afternoon, and once in the evening. If we are unsuccessful or we contact you and you do not schedule we will have no
choice but to cancel your agreement. You will need to be home approximately 20-30 minutes and we will need 30-45 minutes outside
the structure.
If you wish to save up-to $5.00 in fuel surcharges you may pre-pay prior to scheduled service and
pay $________ by mailing no later than 10 days of receiving this notice. You can save an additional 5% by paying online at www.accuratethriftypc.com and
using our convenant pay options located on our customer service page.
Waiting until time of service will add the fuel surcharges to
the aboue total

In order to reinstate your services, you will have to pay your last annual inspection plus a $100.00 reinstatement fee and .50
cents per day for everyday past your anniversary date. Also, there will be late charges and the possibility that you could loose your
repair agreement. Please pay and schedule prior to your anniversary date******

with rental houses, effective immediately, all services are to be paid for prior to each services and their anniversary date.
Sorry for any inconvience this may cause you.
2. Any new additions or changes to the property will not be covered unless it is treated
and a new contract reflects the change in your coverage.
3. Due to the State o Arkansas regulations, your annual inspection must be
completed 30 days proir to your anniversary date.
4. There is Absolutly no coverage unless your account is paid in it's entirity.
This applies to payments, even if you are up to date, your repair warranty is suspended until you are paid in full. If you fail
to pay as you agreed your repair agreement will be cancelled and you8 must be reinspected and pay additional fees, but your service
agrrement will remain in effect with no additional charges.
